What we find

What's usually under a leaking shower

A failed shower pan opened up: water tracking through the old membrane, rot in the framing underneath.

By the time a stain shows up on the ceiling below, the water has been getting past the tile for a long time. The fix is opening the floor, finding the failure, and rebuilding the pan as a system.
